چکیده

mortality among subjects 65 and older with hemoglobin levels even in the “normal range,” is intriguing, in other studies2,3 confounding comorbid conditions accounted for most or all of the difference between anemic and nonanemic groups. Furthermore, even if 22% of the population were at risk for early demise because their hemoglobin levels were found to be low, it would remain to be shown that therapy directed at the hemoglobin level would have any effect on survival. In addition, given that high hemoglobin levels also have deleterious effects on function, it is likely that the normal distribution of hemoglobin represents a balance struck by evolutionary forces. Future studies, some of which we hope to be able to perform, may clarify some of these issues. In the meantime, we believe that physicians will have to content themselves with reference standards based on population norms, as we do with most other clinical laboratory measurements.

LIVER DISEASE The role of iron and haemochromatosis gene mutations in the progression of liver disease in chronic hepatitis C

Background: Chronic hepatitis C virus (HCV) infection is frequently associated with elevated markers of iron stores. Recessively inherited mutations in the HFE gene are responsible for iron accumulation in most cases of hereditary haemochromatosis and may have a role in HCV infection. They may also be associated with progressive liver fibrosis although this remains controversial. Aims: To asses...

Liver iron concentration and fibrosis in a cohort of transfusion-dependent patients on long-term desferrioxamine therapy.

Secondary iron overload is associated with significant mortality and morbidity. Although new, less invasive techniques are becoming available, the most acceptable and readily accessible way to assess iron overload is to measure hepatic iron by liver biopsy. In this study, we report on serial liver biopsies (at least 2) in a cohort of transfusion-dependent patients (49) on long-term desferrioxam...

Relationship between Serum Iron Indices and Hepatic Iron Quantitation in Patients with Fatty Liver Disease

Fatty liver disease can range from fatty liver alone (steatosis) to fatty liver associated with inflammation (steatohepatitis). Purpose: The study was undertaken to determine the relationships between the histological finding and biochemical measures of hepatic injury with hepatic iron content in patients with chronic liver diseases and steatosis. Methods: The study of 115 patients with chronic...

Computed tomography for determining liver iron content in primary haemochromatosis.

Dual-energy computed tomography (CT) was used to estimate hepatic iron concentration in eight patients with primary haemochromatosis with varying degrees of iron overload. The values corresponded closely with these derived from chemical analysis of liver tissue obtained by biopsy (correlation coefficient 0.993). Dual-energy CT therefore seems to provide an accurate and non-invasive alternative ...
